The following attachment shows step-by-step procedures for member agencies to apply for funds to help your service receive H1N1 Pandemic Supplies. Do this today, as the deadline for submission of the application packet is 3:00 pm, September 8, 2009 to the address shown on page 1 of the following documents. The deadline must be met - don't be late.
